Volunteer Accident Waiver and Release of Liability
All volunteers with South Hero Land Trust must read the Volunteer Waiver and Release, and sign an Acknowledgement Form certifying that you have read and agree to the terms.
You may sign the Acknowledgement Form using the form below. You can also print the Downloadable Acknowledgement Form, and email a signed copy to Jenna, or give it to a SHLT staff member when you see us next.
Afterschool Club Accident Waiver and Release of Liability
Before a child will be allowed to participate in a SHLT Afterschool Club, their parent or legal guardian must read the Afterschool Club Waiver and Release Form and certify that they agree to its terms.

Photo Release Form for Minors
Please let us know how we can use photos of your child using the form below. You can also print our Downloadable Photo Release Form and email a signed copy to Jenna or give it to a SHLT staff member when you see us next!
